Total Cancer Care Protocol: A Lifetime Partnership with Patients Who Have or May be at Risk of Having Cancer.

1.1 To establish a longitudinal clinical and related data and tissue repository that will contain:
X Initial demographics, medical history, family history, nutritional history, cancer predisposing risk factors, quality of life, and quality of care data from all participants via surveys, review of medical records and cancer registries. (Required participation element for Consortium Sites)
X Lifetime follow-up information regarding medical treatment, pathology records, and response and recurrence history of all participants as long as the study remains in progress. (Required participation element for Consortium Sites).
